Augtyro (repotrectinib) is a kinase inhibitor indicated for the treatment of:
- adult patients with locally advanced or metastatic ROS1-positive non-small cell lung cancer (NSCLC).
- adult and pediatric patients 12 years of age and older with solid tumors that have a neurotrophic tyrosine receptor kinase (NTRK) gene fusion and are locally advanced or metastatic or where surgical resection is likely to result in severe morbidity, have progressed following treatment or have no satisfactory alternative therapy.
This indication is approved under accelerated approval based on overall response rate and duration of response. Continued approval for this indication may be contingent upon verification and description of clinical benefit in the confirmatory trials.
Recommended Dosage:
The recommended dosage of Augtyro for adult patients and pediatric patients aged 12 years and older is 160 mg administered orally once daily for the first 14 days. Following this initial treatment period, the dosage should be increased to 160 mg administered orally twice daily. Treatment should be continued until disease progression or the occurrence of unacceptable toxicity. Augtyro may be taken with or without food and should be administered at approximately the same time each day.
Capsules must be swallowed whole. Do not open, crush, chew, or dissolve the capsules prior to administration. Capsules that are broken, cracked, or otherwise damaged should not be used. If a dose is missed, or if vomiting occurs after dose administration, the patient should not take an additional dose. The next dose should be taken at the regularly scheduled time.
- Rare cases of interstitial lung disease or pneumonitis may occur. Watch for new or worsening cough, shortness of breath, or fever. Stop treatment and seek medical attention if breathing problems occur. Early evaluation helps prevent serious complications.
- Dizziness, confusion, and seizures have been reported in some patients taking this medication. You should monitor for unusual behavior and changes in mental state. If any symptoms develop, do not drive or operate any kind of machinery. If worsening neurological symptoms develop, tell your doctor right away.
- Liver injury may occur while receiving treatment. Regular liver function tests are required. Symptoms may include fatigue, dark urine, yellowing of the skin or eyes, or abdominal pain. Contact your clinician immediately if signs or symptoms of liver problems appear.
- Tenderness, muscle pain, or weakness may develop, sometimes accompanied by elevated creatine phosphokinase (CPK) levels. Report unexplained muscle discomfort promptly. Your doctor may monitor blood tests and adjust treatment if muscle injury is suspected.
- This medicine may increase the risk of uric acid levels in the blood, leading to hyperuricemia or gout. Monitor for joint pain, swelling, or redness. Periodic blood tests may be required, especially in patients with a history of gout.
- Your treatment may put you at a slightly higher risk for bone fractures. Those with osteoporosis or other conditions that weaken bones should be particularly careful. Tell your doctor if you have a history of fractures or pain in your bones. You may need to increase your calcium and vitamin D intake.
- This medicine could harm your baby. Do not take this medicine during your pregnancy, unless it is necessary. Women who could become pregnant must use birth control during this treatment, and should contact their doctor if they become pregnant.
- It is not known whether this medicine passes into human breast milk. Breastfeeding is generally not recommended during treatment. Discuss risks and benefits with your healthcare provider to determine the safest feeding option for your baby.
