Ekterly (sebetralstat) is a plasma kallikrein inhibitor indicated for the treatment of acute attacks of hereditary angioedema (HAE) in adult and pediatric patients aged 12 years and older.

Recommended Dosage:
The recommended dosage of Sebetralstat is one dose of 600 mg (two tablets) orally at the earliest recognition of an acute HAE attack. A second dose of 600 mg (two tablets) may be taken at least 3 hours after the first dose if the response is inadequate, or if symptoms worsen or recur. The maximum recommended dosage is 1,200 mg (four tablets) in any 24 hours.

• Sebetralstat 300 mg should not be used during pregnancy due to a lack of supporting human evidence. Animal studies have shown fetal harm and loss of fetus during pregnancy at very high doses. Only use this therapeutic drug in case the potential benefits far exceed the probable risks.
• It is not known if sebetralstat or its metabolites are excreted in breast milk. However, animal studies detected radioactivity in rat milk. Thus, breastfeeding is not advised while on the medication unless the therapeutic benefits justify the risks to the infant.
• Clinical trials show that the safety and efficacy of sebetralstat are only confirmed for patients 12 years and older. No data support its use in children aged under 12 years. Therefore, sebetralstat should not be prescribed to pediatric patients under 12 years old.
• Only a tiny number of people aged 65 and above were part of the clinical trials, so it is not specified whether older adults react differently from younger ones. Clinicians should closely watch when treating older patients, but there’s no special dose adjustment suggested at this time.
• No dose adjustment is required for those with mild or moderate hepatic impairment (Child-Pugh A or B). Use of this therapeutic drug in severe hepatic impairment (Child-Pugh C) is not required, as exposure may markedly increase and compromise safety.
