Medical Uses
CARDAMYST (etripamil), from Milestone Pharmaceuticals, is the first self-administered nasal spray approved by the FDA to convert episodes of paroxysmal supraventricular tachycardia (PSVT) to normal sinus rhythm in adults. It lets patients treat an episode wherever it happens instead of going to the emergency department.
Etripamil is a short-acting L-type calcium channel blocker that slows conduction through the AV node, interrupting the re-entry circuit behind PSVT. In the Phase 3 RAPID trial, 64% of patients returned to normal rhythm within 30 minutes versus 31% on placebo. One device (70 mg) is used at symptom onset; a second dose may be taken after 10 minutes if symptoms persist, up to 140 mg in 24 hours.
