Cystagon (cysteamine bitartrate) is used for the management of nephropathic cystinosis in pediatric and adult patients. Nephropathic cystinosis is a rare genetic disorder characterized by the abnormal accumulation of cystine crystals in various organs, particularly the kidneys. Cystagon works by reducing cystine levels in cells, thereby slowing the progression of kidney damage and preserving renal function. 

Recommended Dosage: 

For children up to age 12 Years: The dosing should be based on the patient’s body surface area (g/m2 /day). The recommended dose is 1.30 g/m2 /day of the free base divided 4 times daily.

For patients over age 12 and over 50 kg weight: The recommended dose is 2 g/day, divided 4 times daily. Starting doses need to be 1/4 to 1/6 of the expected maintenance dose, increased gradually over 4 to 6 weeks to avoid intolerance. The dose should be increased if there is adequate tolerance and the leucocyte cystine level remains >1 nmol hemicystine/mg protein. Avoid doses higher than 1.95 g/m2 /day, as it is not recommended.

• Cystagon 50 mg/150 mg can lead to bone marrow suppression, resulting in decreased production of blood cells, including red blood cells, white blood cells, and platelets. Patients should undergo regular blood tests to monitor their blood cell counts, and dosage adjustments may be necessary if significant decreases are observed.
  
• Gastrointestinal side effects, such as nausea, vomiting, abdominal pain, and diarrhea, are common with Cysteamine therapy. Patients should be advised to take the medication with food or milk to minimize these effects. Severe or persistent gastrointestinal symptoms should be reported to a healthcare provider promptly.
  
• This medicinal product may cause central nervous system (CNS) side effects, including dizziness, headache, and seizures, particularly at higher doses. Patients should be cautious when performing activities that require mental alertness, such as driving or operating machinery. Any signs of CNS toxicity should be reported to a healthcare provider immediately.
  
• Skin reactions, such as rash, itching, and erythema (redness), may occur with Cysteamine 50 mg/150 mg therapy. Patients should avoid excessive sun exposure and use sunscreen to protect their skin. If severe or persistent skin reactions occur, medical attention should be sought.
  
• This medicine may cause ocular side effects, including increased intraocular pressure (glaucoma) and corneal cystine crystal deposition. Regular eye examinations are recommended during treatment to monitor for these effects. Patients should report any changes in vision or eye discomfort to their healthcare provider.
  
• Cystagon can affect renal function and may lead to kidney damage or failure, especially if not properly managed. Regular monitoring of kidney function, including serum creatinine and urine protein levels, is essential during treatment. Dosage adjustments may be necessary based on renal function tests.
  
• Long-term use of this therapeutic drug has been associated with an increased risk of hypothyroidism (underactive thyroid). Thyroid function should be monitored regularly, and thyroid hormone replacement therapy may be required if hypothyroidism develops.
  
• Cystagon should not be used during pregnancy unless the potential benefits outweigh the risks to the fetus. Women of childbearing age should use effective contraception during treatment. It is not known whether Cystagon is excreted in breast milk, so breastfeeding should be avoided while taking the medication.
