ISEMBYLD (apitegromab-mstn), from Scholar Rock, is the first spinal muscular atrophy (SMA) therapy that targets muscle rather than motor neurons. The FDA approved it on 11 September 2026 for adults and children aged 2 years and older with SMA who are already receiving an SMN2-targeted treatment (such as nusinersen or risdiplam).
Apitegromab blocks the activation of myostatin, a natural protein that limits muscle growth, helping preserve and build muscle strength on top of SMN therapy. In the Phase 3 SAPPHIRE trial, 34.2% of treated patients gained at least 3 points on the Hammersmith motor scale (HFMSE) at 52 weeks versus 13.5% on placebo. The recommended dose is 10 mg/kg by IV infusion once every 4 weeks.
