Dordaviprone is a small-molecule imipridone that selectively activates the mitochondrial protease ClpP and antagonizes dopamine receptor D2, disrupting mitochondrial function and cell signaling pathways in tumor cells carrying the H3 K27M mutation, leading to tumor cell death.

• QT interval prolongation — ECG monitoring is recommended before and during treatment; avoid concomitant use with other QT-prolonging drugs.
• Hepatotoxicity has been reported; monitor liver function tests periodically.
• Common side effects include fatigue, nausea, vomiting, headache, and elevated liver enzymes.
• This indication is approved under accelerated approval based on response rate; continued approval may be contingent on confirmatory trials.
