Tarpeyo (budesonide) delayed-release capsules, from Calliditas Therapeutics, are indicated to reduce the loss of kidney function in adults with primary immunoglobulin A nephropathy (IgAN) at risk of rapid disease progression.

Budesonide is a corticosteroid formulated for targeted release in the ileum, where it acts locally on Peyer’s patches to reduce the production of pathogenic IgA1 antibodies that drive IgA nephropathy, while minimizing systemic corticosteroid exposure.

• Hypercorticism and adrenal suppression may occur, especially with concomitant use of CYP3A4 inhibitors.
• Increased risk of infections due to immunosuppressive effects of corticosteroids.
• Should be used with caution in patients with hypertension, diabetes, osteoporosis, or peptic ulcers.
• Should not be discontinued abruptly after prolonged use — taper as directed by the treating physician.
