VUMERITY (diroximel fumarate), from Biogen, is indicated for the treatment of relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease, in adults.

Diroximel fumarate is converted in the body to monomethyl fumarate, which is thought to activate the Nrf2 pathway, an antioxidant and anti-inflammatory pathway that helps protect nerve cells from oxidative stress and inflammation in MS.

• Progressive multifocal leukoencephalopathy (PML) has been reported with fumarate-class drugs; monitor for any new neurological symptoms.
• Lymphopenia (low lymphocyte count) may occur; monitor complete blood counts before and during treatment.
• Flushing and gastrointestinal reactions (nausea, diarrhea, abdominal pain) are commonly reported, generally less than with dimethyl fumarate.
• Anaphylaxis and angioedema have been reported.
• Liver injury has been reported; monitor liver function.
