Winrevair (sotatercept-csrk) is an activin signaling inhibitor used for the treatment of adult patients with pulmonary arterial hypertension (PAH, WHO Group 1) to increase exercise potential, improve WHO functional class (FC) and scale down the probability of clinical worsening events.
Recommended Dosage: Winrevair is administered once every three weeks by subcutaneous injection according to the patient’s body weight. The recommended starting dose is 0.3 mg/kg and the target dose is 0.7 mg/kg. Dosage modifications because of increased hemoglobin (Hgb) and decreased platelets may be important. Check Hgb and platelets before each dose for the initial five doses, or longer if values are unstable, and monitor periodically thereafter.
If a dose is missed, administer it as quickly as possible. If the missed dose is not administered within 3 days of the scheduled date, adjust the schedule to maintain three-week dosing intervals. In case of an overdose, monitor for erythrocytosis.
- Treatment with Sotatercept 45 mg/60 mg may increase hemoglobin. Severe erythrocytosis may increase the probability of thromboembolic events or hyperviscosity syndrome. Monitor Hgb before each dose for the initial five doses, or longer in case values are unstable, and periodically thereafter, to specify if dose adjustments are required.
- Winrevair may decrease platelet count. Serious thrombocytopenia may increase the probability of bleeding. Do not start treatment if the platelet count is <50,000/mm3. Monitor platelets prior to each dose for the first 5 doses, or longer if values are unstable, and periodically thereafter to specify whether dose adjustments are needed.
- Serious bleeding (e.g., gastrointestinal, intracranial hemorrhage) may occur in patients taking treatment with Sotatercept-csrk. Assess and treat bleeding accordingly. Avoid administering this medicine if the patient is experiencing severe bleeding.
- Winrevair may be responsible for causing fetal harm when administered to a pregnant woman. e Females of reproductive age/potential should use an effective method of contraception during treatment and for at least four months after the final dose.
