What is Lysodren used for?
Lysodren (mitotane) is an oral adrenal cytotoxic agent. It acts directly on the adrenal cortex, suppressing it and modifying the peripheral metabolism of steroids, which reduces the excess hormone production and tumour activity seen in adrenocortical carcinoma. (Its exact mechanism of action is not fully understood.)
It is approved for:
- Inoperable, functional or nonfunctional, adrenocortical carcinoma (ACC) — cancer of the adrenal cortex.Lysodren is described as the first and only FDA-approved treatment for ACC. Before starting, a pelvic ultrasound (in premenopausal women), liver function tests, and a complete blood count are evaluated. It is a hazardous cytotoxic drug and a prescription anticancer medicine that must be used under the supervision of a qualified specialist.
Recommended dosage: The recommended initial dose of Lysodren is 2000 mg to 6000 mg orally per day, in three or four divided doses, taken with food. The dose is then titrated by the treating doctor, guided by plasma mitotane levels (a target of approximately 14–20 mg/L) and tolerability. Mitotane is lipophilic and accumulates in fatty tissue.
Because Lysodren suppresses the adrenal glands, patients usually require corticosteroid (and sometimes mineralocorticoid) replacement during treatment, and higher-than-usual steroid replacement doses may be needed. Caregivers are advised to wear disposable gloves when handling the tablets. The treating specialist always determines the exact dose and any modifications.
Boxed warning — adrenal crisis in the setting of shock, severe trauma, or infection: patients treated with Lysodren are at increased risk of developing adrenal crisis, which can be fatal. If shock, severe trauma, or infection occurs, Lysodren is temporarily discontinued, and exogenous steroids are given. Patients are monitored closely for infection and instructed to contact their physician immediately if injury, infection, or another illness occurs.
- Adrenal insufficiency: Lysodren can cause or worsen adrenal insufficiency; glucocorticoid and mineralocorticoid replacement is monitored and adjusted (free cortisol and ACTH may be tracked).
- CNS effects: dizziness, drowsiness, and other central nervous system effects can occur and may affect the ability to drive or operate machinery.
- Hepatotoxicity and blood count changes: liver function and blood counts are monitored.
Ovarian macrocysts in premenopausal women; a baseline pelvic ultrasound is recommended. - Drug interactions: mitotane induces liver enzymes and can reduce the effect of many drugs; concomitant use with warfarin should be avoided (or INR monitored closely).
- Embryo-fetal toxicity: Lysodren can cause fetal harm, including preterm birth and early pregnancy loss; a pregnancy test is done before treatment and effective contraception is advised.
Common side effects:
Common side effects include nausea, vomiting, diarrhoea, loss of appetite, drowsiness, dizziness, lethargy, and skin rash. This is not a complete list — refer to the full prescribing information and your doctor.
Patients in India can legally access Lysodren through the Named Patient Program (NPP) — an import route that allows a medicine not yet registered in India to be imported in an individual patient’s name, on their physician’s prescription. The process is simple and fully documented:

- Eligibility & document check — we verify the prescription, diagnostic reports, and patient ID against regulatory requirements.
- Receive your quote — a transparent quotation covering medicine cost, logistics, and timeline.
- Regulatory approvals — we assist with the import permit and documentation needed for legal cross-border access.

Documents required to import Lysodren:

- The sourcing country and current ex-market price (USA, Europe, Canada, Japan, China, or Australia).
- Logistics, insurance, and any applicable customs duties or import permit costs.
