Cardioxane (dexrazoxane) is a cardioprotective agent designed to help prevent the long-term heart damage caused by chemotherapy drugs like doxorubicin or epirubicin in adults with advanced and/or metastatic breast cancer. It’s particularly used for patients who have already received a cumulative dose of 300 mg/m² of doxorubicin or 540 mg/m² of epirubicin and need further chemotherapy.
Recommended Dosage: Cardioxane should be administered as a short intravenous (IV) infusion over 15 minutes, approximately 30 minutes before the administration of anthracyclines. This timing allows for a 15-minute gap between treatments. The recommended dose for Cardioxane injection is based on the anthracycline dose: it should be 20 times the doxorubicin dose or 10 times the epirubicin dose. Therefore, when using the standard dosage of doxorubicin at 50 mg/m² or epirubicin at 100 mg/m², Cardioxane should be given at a dose of 1000 mg/m². If the anthracycline dose is reduced for any reason, such as liver impairment, the Cardioxane dose should be adjusted accordingly.
- Treatment with Dexrazoxane 500 mg injection can cause myelosuppression, leading to reduced bone marrow activity and decreased production of blood cells. Patients should be closely monitored for signs of anemia, leukopenia, and thrombocytopenia. Regular blood counts are essential, and dose adjustments may be necessary to manage severe bone marrow suppression.
- There is an increased risk of developing second primary malignancies, such as acute myeloid leukemia and myelodysplastic syndrome, when using Dexrazoxane. Patients should be informed of this risk, and long-term monitoring is recommended for early detection and management.
- The medicine Dexrazoxane may interfere with the efficacy of certain chemotherapy agents, particularly anthracyclines. Healthcare professionals should evaluate the potential benefits and risks when using this therapeutic drug alongside chemotherapy and adjust treatment plans accordingly.
- Treatment with this medicinal product can cause liver toxicity, manifesting as elevated liver enzymes or liver damage. Patients with pre-existing liver conditions require careful monitoring and potential dose adjustments. Regular liver function tests are recommended during treatment.
- Patients with pre-existing cardiac disorders should use Cardioxane 500 mg injection with caution, as it may exacerbate heart conditions. Cardiac function should be monitored closely, particularly in patients receiving high cumulative doses of anthracyclines.
- There is a risk of thromboembolic events associated with this treatment. Patients with a history of thromboembolism should be closely monitored, and preventive measures should be considered.
- Cardioxane may harm the fetus and is not recommended for pregnant women. Women of child-bearing potential should use effective contraception during treatment and for a specified period afterward. Men receiving Cardioxane should also use contraception to prevent potential harm to the fetus.
