Fetroja (cefiderocol)
- Medicine Name: Fetroja
- API: Cefiderocol
- Dosage Form & Strength: Injection: 1 gram of cefiderocol as a lyophilized powder for reconstitution in single-dose vials
- Manufactured By: Shionogi & Co., Ltd.
Fetroja (cefiderocol) is a cephalosporin antibacterial indicated in patients 18 years of age or older for the treatment of the following infections caused by susceptible Gram-negative microorganisms:
- Complicated Urinary Tract Infections (cUTI), including Pyelonephritis.
- Hospital-acquired Bacterial Pneumonia and Ventilator-associated Bacterial Pneumonia (HABP/VABP).
To reduce the development of drug-resistant bacteria and maintain the effectiveness of Fetroja and other antibacterial drugs, Fetroja should be used only to treat or prevent infections that are proven or strongly suspected to be caused by bacteria.
Recommended Dosage:
The recommended dosage of Fetroja is 2 grams administered every 8 hours by intravenous (IV) infusion over 3 hours in adults with a creatinine clearance (CLcr) of 60 to 119 mL/min. Dosage adjustment is recommended for patients with CLcr less than 60 mL/min, including patients receiving intermittent hemodialysis (HD) or continuous renal replacement therapy (CRRT), and for patients with CLcr 120 mL/min or greater. The recommended duration of treatment with Fetroja is 7 to 14 days. The duration of therapy should be guided by the patient’s clinical status.
- Studies have shown higher death rates in patients with serious infections treated with
- Fetroja compared to other treatments. It should be used only when no better options are available, especially in severe carbapenem-resistant infections, after careful medical evaluation.
Fetroja may cause allergic reactions, including serious ones like rash, itching, swelling, or difficulty breathing. Patients with a history of allergy to beta-lactam antibiotics should use it with caution. Immediate medical help is needed if any reaction occurs. - Use of antibiotics like Fetroja can cause diarrhea caused by Clostridioides difficile. This condition can range from mild to severe and may occur even after treatment ends. Patients should contact a doctor if persistent diarrhea or stomach pain develops.
- Fetroja may increase the risk of seizures or confusion, especially in patients with kidney problems or existing neurological conditions. Dose adjustment may be required. Any unusual symptoms like confusion, twitching, or seizures should be reported immediately to a doctor.
- Using Fetroja unnecessarily or incorrectly can lead to bacteria becoming resistant to antibiotics. It should only be used for confirmed or strongly suspected bacterial infections. Completing the full prescribed course helps reduce the risk of resistance development.
- There is limited data on the use of Fetroja during pregnancy. It should only be used if clearly needed and prescribed by a doctor after evaluating potential risks and benefits. Pregnant women should inform their healthcare provider before starting treatment.
- It is not known if this medicine passes into human breast milk. Caution is required when used by breastfeeding women. The doctor may evaluate whether to stop breastfeeding or avoid the medicine based on the importance of the treatment for the mother.
