Estramustine phosphate combines an estrogen (estradiol) with a nitrogen mustard alkylating agent. It concentrates in prostate tissue and is thought to act primarily through binding to microtubule-associated proteins, disrupting cell division, along with hormonal (anti-androgenic) effects.

• Increased risk of thromboembolic events (blood clots), fluid retention, and cardiovascular complications due to its estrogenic component.
• Use with caution in patients with a history of clotting disorders, heart disease, or hypertension.
• Liver function should be monitored during treatment.
• Common side effects: nausea and vomiting, gynecomastia (breast enlargement/tenderness), fluid retention, decreased libido/sexual dysfunction.
