Erwinase (Crisantaspase)

  • Medicine Name: Erwinase
  • API: Crisantaspase (L-asparaginase from Erwinia chrysanthemi)
  • Dosage Form & Strength: Powder for injection/infusion: 10,000 international units (IU)/3 mL
  • Manufactured By: Porton Biopharma Ltd.

Medical Uses

Erwinase (crisantaspase) is an L-asparaginase enzyme derived from the bacterium Erwinia Chrysanthemi that is used for the treatment of a cancer of the white blood cells (WBCs) called Acute Lymphoblastic Leukaemia (ALL), a type of cancer that particularly affects children aged 4 months and above, who have developed allergic reactions to E.coli derived asparaginase. Erwinase may be used alone or in combination with other treatments.

Recommended Dosage:
To substitute for a dose of pegaspargase: The recommended dose for each planned dose of pegaspargase is 25,000 international units/m2 administered intramuscularly or intravenously three times a week (Monday/Wednesday/Friday) for two consecutive weeks (total 6 doses). The same dosage recommendations apply to both paediatric and adult patients.

To substitute for a dose of native E. coli asparaginase: The recommended dose of Erwinase is 25,000 International Units/m2 administered intramuscularly or intravenously for each scheduled dose of native E. coli asparaginase within a treatment.

When administering this therapeutic drug intravenously, the desired nadir (pre-dose) serum asparaginase activity (NSAA) levels may not be achieved; consider monitoring NSAA levels and switching to intramuscular (IM) administration if the desired NSAA levels are not achieved with intravenous (IV) dosing.

Warning & Precautions

  • Rash, swelling, difficulty breathing, or shock may all occur as suddenly as serious allergic reactions. Every patient should be monitored during and after each dose. After taking the injection, inform me immediately if you feel unwell or notice signs of allergy that lead to additional complications.
  • Crisantaspase 10,000 IU (L-asparaginase from Erwinia chrysanthemi) could lead to inflammation of the pancreas, which may result in severe or even life-threatening conditions. Stop treatment if symptoms appear, and contact your healthcare provider right away for evaluation and further medical assistance that may not be needed.
  • During treatment, some patients may experience an increase in fatigue, thirst, urination, or even confusion, which are all signs of high blood sugar. Routinely checking blood sugar levels is highly recommended, and treatment adjustments may be needed.
  • There is an increased risk of bleeding and blood clots while a patient is on crisantaspase. Symptoms include: discomfort in the legs, chest pain, difficulty breathing, or bruising that is different from normal. The patient must seek medical attention right away during treatment if any of these signs occur.
  • This therapeutic drug may cause harm to an unborn baby. Do not use it during pregnancy unless absolutely necessary. Women of childbearing age should use appropriate contraception during treatment and consult their healthcare provider (HCP) if they become pregnant or are looking to conceive soon.
  • It is unclear whether Crisantaspase actively enters the breast milk. It may pose a risk to the baby, so breastfeeding is contraindicated during the treatment period. Discuss with your physician whether to cease breastfeeding or this medicine, considering the benefits and risks.
  • Using this medicine could influence fertility in males and females. Women and men planning to conceive should see a physician before starting the treatment. Other options for preserving fertility may be considered. It is crucial to comprehend the long-term implications on reproductive health.
  • Treatment with Crisantaspase 10,000 IU injection may cause side effects like confusion, dizziness, or fatigue. These can impair the potential to drive or operate machinery safely. Avoid such activities in case you experience these signs and symptoms. Always consider the impacts of the medicine on your condition prior to engaging in hazardous tasks.
  • Erwinase contains traces of sodium, and Sodium as well as Glucose. Treating physicians should take this into account in patients with diabetes or heart problems who require sodium or sugar restriction.
