FILSPARI (Sparsentan) Tablets

FILSPARI (sparsentan), from Travere Therapeutics, is a dual endothelin and angiotensin II receptor antagonist indicated to reduce proteinuria in adults with primary IgA nephropathy (IgAN) who are at risk of rapid disease progression (urine protein-to-creatinine ratio ≥1.5 g/g).

FILSPARI blocks both endothelin type A receptors and angiotensin II type 1 receptors, two pathways involved in glomerular injury and proteinuria in IgA nephropathy, reducing protein leakage and slowing kidney function decline.

• Hepatotoxicity: liver enzymes must be checked before starting and periodically during treatment.
• Embryo-fetal toxicity: available only through a restricted REMS-style program in the US due to birth-defect risk; effective contraception is required.
• Fluid retention and hypotension can occur due to its dual receptor-blocking action.
• Common side effects: peripheral edema, hypotension, dizziness, hyperkalemia, elevated liver enzymes.
