KRYSTEXXA (Pegloticase) Injection

KRYSTEXXA (pegloticase), from Horizon Therapeutics (now Amgen), is indicated for the treatment of chronic gout refractory to conventional therapy in adult patients.

Pegloticase is a PEGylated uric acid-specific enzyme that converts uric acid into allantoin, a water-soluble compound that is more easily excreted by the kidneys, helping to rapidly lower serum uric acid levels in patients who have not responded to standard urate-lowering therapies.

• Boxed warning: anaphylaxis and infusion reactions can occur; premedication and administration in a healthcare setting with resuscitation equipment is required.
• Should not be co-administered with other urate-lowering therapies, which can mask the loss of response and increase risk of reaction.
• Screen for G6PD deficiency before starting treatment — pegloticase is contraindicated in G6PD-deficient patients due to risk of hemolysis and methemoglobinemia.
• Gout flares are common during the first few months of treatment as uric acid levels drop.
