Pentacarinat (pentamidine) is a broad-spectrum anti-infective agent active against several parasitic worms, protozoa, and fungi. It is used in the treatment and prevention of Pneumocystis jirovecii (carinii) pneumonia in high-risk, HIV-infected patients who meet the following criteria:
- A history of one or more previous episodes of Pneumocystis jiroveci (carinii) pneumonia
- A CD4+ count of 200 cells/mm^3 or less
Because of its various toxicities, its use is only for severe Pneumocystis jirovecii (carinii) pneumonia, which has failed to respond to other first-line agents.
Recommended Dosage: The recommended dosage is 4 mg/kg body weight administered once daily for at least 2 to 3 weeks (14 to 21 days), preferably by slow intravenous (IV) infusion, as intramuscular (IM) administration is not recommended for the treatment of Pneumocytosis jiroveci (carinii) pneumonia.
- The medicine Pentamidine should be used with particular caution in those with hepatic and/or renal dysfunction, hyperglycemia or hypoglycemia, hypertension or hypotension, leucopenia, thrombocytopenia, or anemia.
- Fatalities because of serious hypotension, hypoglycaemia, acute pancreatitis and cardiac arrhythmias may occur in those treated with pentamidine, by both the IV and IM routes. Blood pressure must be closely monitored during administration and at regular intervals until therapy is concluded.
- Monitor fasting blood glucose measurements daily throughout therapy, and at regular intervals after the completion of therapy. Hyperglycaemia and diabetes mellitus, with or without preceding hypoglycaemia may occur up to several months following the cessation of therapy.
- Monitor blood urea nitrogen, serum creatinine, urine analysis, serum electrolytes, and complete blood and platelet counts daily during therapy. Also monitor liver function tests (LFTs) including bilirubin, aspartate aminotransferase (AST/SGOT), alkaline phosphatase, and alanine aminotransferase (ALT/SGPT).
