Ofev (Nintedanib) is a kinase inhibitor used in adults for:

    Recommended Dosage:
    The recommended dosage of Nintedanib is 150 mg taken orally twice daily, administered approximately 12 hours apart. Nintedanib capsules should be taken with food and swallowed whole with liquid. The capsules should not be chewed because of the bitter taste. Capsules should not be opened or crushed.If contact with the content of the capsule occurs, wash hands immediately and thoroughly. The effect of chewing or crushing the capsule on the pharmacokinetics of nintedanib is not known.

If a dose of Nintedanib is missed, the next dose should be taken at the next scheduled time. Advise the patient not to make up for a missed dose. Do not exceed the recommended maximum daily dosage of 300 mg.

  • Do not use this therapeutic drug in those with moderate or serious hepatic impairment. Patients with liver dysfunction may experience increased exposure to the medicine. This may lead to a higher risk of side effects. Liver function must be monitored before and throughout treatment.
  • Treatment with this medicine can cause elevations in ALT, AST, or bilirubin, which may indicate liver injury. Monitor liver enzymes often. Temporarily halt, reduce dose, or discontinue treatment if significant liver abnormalities occur. Educate patients to report signs and symptoms of jaundice or dark-colored urine promptly.
  • Vomiting, diarrhea, and nausea are common with Nintedanib 150 mg/100 mg and may need supportive care or dose modification. Persistent symptoms can cause dehydration and electrolyte imbalance. Patients need to maintain hydration and inform healthcare providers (HCPs) in case symptoms are serious or persistent.
  • Treatment with Nintedanib may cause harm to the developing fetus. It is contraindicated during pregnancy. Females of reproductive age should use effective contraception during treatment and for at least 90 days after the final dose. Confirm pregnancy status before initiating this therapy.
  • Serious arterial events, including myocardial infarction, may occur with Nintedanib treatment. Patients with a history of cardiovascular risk factors require close monitoring. Stop treatment if signs of thromboembolism develop. Patients should report shortness of breath, chest pain, or stroke symptoms promptly.
  • This medicine increases the risk of bleeding, particularly in those taking anticoagulants. Use it with caution in those with past bleeding disorders or on medicines that affect coagulation. Monitor closely for signs and symptoms of bleeding and adjust treatment accordingly if any serious bleeding occurs.
  • Cases of gastrointestinal perforation, some fatal, may occur. Use this medicine with caution in patients with a known history of abdominal surgery or gastrointestinal conditions that may increase the risk. Quickly evaluate any symptoms like serious abdominal pain, and stop therapy if perforation is suspected.
  • Significant proteinuria, including nephrotic syndrome, may occur with the use of Nintedanib. Monitor urine protein levels and renal function periodically. Stop treatment in case patients develop nephrotic-range proteinuria or significant renal impairment, and refer to a nephrologist for further evaluation and management.
