Anktiva (nogapendekin alfa inbakicept-pmln) is a medicine that works like a natural protein called interleukin-15 (IL-15). It is used together with a vaccine called Bacillus Calmette-Guérin (BCG) to treat adults who have a type of bladder cancer that hasn’t responded to BCG treatment. This cancer is called nonmuscle invasive bladder cancer (NMIBC) with carcinoma in situ (CIS), and it may or may not have small growths called papillary tumors.
Recommended Dosage:
For induction: The recommended dosage is 400 mcg administered intravesically with BCG once weekly for 6 weeks. A second induction course may be administered if complete response is not achieved at month 3.
For maintenance: The recommended dosage is 400 mcg administered intravesically with BCG once weekly for 3 weeks at months 4, 7, 10, 13, and 19. For patients with an ongoing complete response at month 25 and later, additional maintenance instillations with BCG may be administered once weekly for 3 weeks at months 25, 31, and 37.
- Patients may experience severe hypersensitivity reactions, including anaphylaxis. Monitor for signs of allergic reactions, such as rash, itching, swelling, or difficulty breathing. Immediate medical attention is necessary if these symptoms occur.
- Infusion-related reactions, such as fever, chills, and hypotension, may occur during or after administration. Patients should be monitored during and for a period after the infusion. Pre-medication may be required to mitigate these effects.
- Anktiva 400 mcg/0.4 mL can increase the risk of serious infections, including bacterial, fungal, and viral infections. Patients should be monitored for signs of infection and treated promptly. Use caution in patients with a history of recurrent infections.
- Patients with hepatic impairment should be closely monitored, as Anktiva may affect liver function. Regular liver function tests are recommended, and dosage adjustments may be necessary based on the severity of liver impairment.
- Anktiva can potentially exacerbate pre-existing autoimmune disorders. Patients with a history of autoimmune conditions should be closely monitored for worsening symptoms or new autoimmune phenomena.
- Cardiovascular events, such as hypertension and thromboembolic events, have been reported. Monitor patients for signs of cardiovascular issues, and manage according to clinical practice guidelines.
- There is limited data on the use of Anktiva in pregnant or breastfeeding women. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Breastfeeding women should consider the potential risks and benefits before using this medication.
