Lenmeldy (atidarsagene autotemcel) is an autologous hematopoietic stem cell-based gene therapy used for the treatment of children with pre-symptomatic late infantile (PSLI), pre-symptomatic early juvenile (PSEJ) or early symptomatic early juvenile (ESEJ) metachromatic leukodystrophy (MLD).
- Recommended Dosage: Lenmeldy is provided as a single dose for infusion containing a suspension of CD34+ cells in one to eight infusion bags. The dose administered is calculated on behalf of the child’s weight at the time of infusion.
- For MLD Subtype Pre-symptomatic late infantile: The minimum recommended dose (CD34+ cells/kg) is 4.2 x 106 and the maximum recommended dose (CD34+ cells/kg) is 30 x 106 administered intravenously.
- For MLD Subtype Pre-symptomatic early juvenile: The minimum recommended dose (CD34+ cells/kg) is 9 x 106 and the maximum recommended dose (CD34+ cells/kg) is 30 x 106 administered intravenously.
- For MLD Subtype Early symptomatic early juvenile: The minimum recommended dose (CD34+ cells/kg) is 6.6 x 106 and the maximum recommended dose (CD34+ cells/kg) is 30 x 106 administered intravenously.
- Lenmeldy may cause infusion-related reactions, such as fever, chills, nausea, vomiting, and hypotension (low blood pressure). Patients should be closely monitored during infusion, and appropriate supportive care should be readily available.
- Treatment with Lenmeldy injection may lead to hematologic toxicity, including leukopenia, thrombocytopenia, and neutropenia. Regular monitoring of complete blood counts is essential, and treatment should be interrupted or discontinued as necessary to manage hematologic toxicity.
- Patients receiving Lenmeldy are at increased risk of developing infections, including bacterial, viral, fungal, and protozoal infections. Prophylactic antibiotics or antifungal agents may be considered, and prompt treatment of infections is essential.
- Lenmeldy contains genetically modified cells and may cause GvHD, a potentially life-threatening complication characterized by immune-mediated damage to host tissues. Patients should be monitored for signs and symptoms of GvHD, and appropriate treatment should be initiated if GvHD occurs.
- This therapeutic drug may be responsible for causing TLS, a metabolic complication characterized by the rapid release of intracellular contents into the bloodstream. Patients should receive hydration and electrolyte monitoring to prevent and manage TLS.
- Treatment with this medicine may result in hepatic toxicity, including elevated liver enzymes and hepatic impairment. Liver function tests should be monitored regularly, and treatment should be interrupted or discontinued as necessary to manage hepatic toxicity.
