Lupkynis is a calcineurin-inhibitor immunosuppressant used in combination with a background immunosuppressive therapy regimen for the treatment of adult patients with active lupus nephritis (LN).
Recommended Dosage: The recommended starting dose is 23.7 mg twice daily. Use lupkynis capsules together with mycophenolate mofetil (MMF) and corticosteroids. Capsules must be swallowed completely and must not be crushed, opened, or divided. Take it on an empty stomach persistently as close to a 12-hour schedule as possible, and with a minimum of 8 hours between doses.
If a dose is missed, the patient should take it as soon as possible within 4 hours after missing the dose. Beyond the 4-hour time frame, patients should wait until the usual scheduled time to take the next regular lupkynis dose. Patients should not double the next dose of lupkynis.
- Patients should be evaluated for skin changes and advised to avoid or limit sun exposure and to avoid artificial UV light (sun lamps, tanning beds) by wearing protective clothing and using a broad spectrum sunscreen (SPF 30 or higher).
- Immunosuppressants, including Lupkynis 7.9 mg capsules, increase the risk of having bacterial, fungal, viral, and protozoal infections, including opportunistic infections. Monitor for infection. Consider the benefits/risks for patients; use the lowest effective dosage required to maintain response.
- Voclosporin, like other calcineurin-inhibitors, can cause acute or chronic nephrotoxicity. Evaluate eGFR on a regular interval during treatment, and consider dose reduction or interruption in patients with reductions in eGFR; persistent decrease of eGFR needs to be assessed for chronic calcineurin-inhibitor nephrotoxicity.
- Hypertension is a common side effect due to the therapy with lupkynis 7.9 mg capsule and may require antihypertensive therapy. Monitor blood pressure on a regular basis during therapy and treat new-onset hypertension and exacerbations of pre-existing hypertension.
- Voclosporin, like other calcineurin-inhibitors, may be responsible for causing a spectrum of neurotoxicities. Assess for neurologic symptoms and consider dose reduction or interruption of Voclosporin therapy in case neurotoxicity occurs.
- Hyperkalemia, which may be severe and require treatment, has been observed with calcineurin-inhibitors including therapy with Lupkynis 7.9 mg Capsules. Assess serum potassium levels periodically during therapy.
- Avoid the use of live attenuated vaccines during treatment with Voclosporin (e.g., BCG, intranasal influenza, mumps, measles, rubella, oral polio, varicella, yellow fever, and TY21a typhoid vaccines). Inactivated vaccines observed to be safe for administration may not be sufficiently immunogenic during therapy with Voclosporin.
- Female patients of the potential risk to a fetus should avoid use of Voclosporin during pregnancy. Females should inform their health specialist if they are pregnant or become pregnant. Women should not breastfeed during treatment with Voclosporin and for 7 days after the last dose of Voclosporin.
