Talvey (talquetamab-tgvs) is a bispecific GPRC5D-directed CD3 T-cell engager used for the treatment of adult patients with relapsed or refractory multiple myeloma (MM) who have received at least four prior lines of therapy, including a proteasome inhibitor (PI), an immunomodulatory agent and an anti-CD38 monoclonal antibody.
Recommended Dosage: The recommended dosages of Talvey subcutaneous (SC) injection are: step-up dose 1 of 0.01 mg/kg on Day 1, step-up dose 2 of 0.06 mg/kg on Day 4, followed by the first treatment dose of 0.4 mg/kg on Day 7, and then 0.4 mg/kg once weekly thereafter.
- Treatment with Talvey can cause cytokine release syndrome, including life-threatening or fatal reactions. Start therapy with step-up dosing and administer pre-treatment medicines (such as corticosteroids, antihistamine, and antipyretics) before each dose in the step-up dosing schedule to reduce the potential of CRS. Monitor patients after administration accordingly.
- Talvey 3 mg/1.5 mL injection can cause life-threatening, or fatal neurologic toxicity, including ICANS. Assess patients for symptoms of neurologic toxicity during therapy. At the initial sign of neurologic toxicity, including ICANS, promptly evaluate the patient and provide supportive care on behalf of severity; withhold or permanently stop treatment on behalf of severity.
- Talquetamab-tgvs can cause weight loss, and oral toxicities, including dry mouth, dysgeusia, dysphagia, and stomatitis. Monitor patients for the symptoms of oral toxicity. Monitor weight regularly throughout therapy. Evaluate clinically significant weight loss further. Withhold therapy or permanently stop on behalf of severity.
- This medicine can cause critical infections, including life-threatening or fatal infections. Monitor patients for symptoms of infection before and throughout treatment and treat appropriately. Administer prophylactic antimicrobials as per local guidelines. Withhold or consider permanent discontinuation of treatment as recommended on behalf of severity.
- Talvey injection can cause serious skin reactions, including erythema, rash, maculo-papular rash, and erythematous rash. Monitor for skin toxicity, including rash progression. Consider early intervention and treatment to manage skin toxicity. Withhold therapy is recommended based on severity.
