Ventavis (Iloprost) is a synthetic analog of prostacyclin used for the treatment of adults with certain kinds of severe pulmonary arterial hypertension (PAH), a condition in which blood pressure is too high in the blood vessels between the heart and the lungs. This therapeutic drug may improve the ability to exercise and symptoms for a short time by lowering blood pressure and opening up the blood vessels in the lungs. Iloprost trometamol has not been studied in children aged younger than 18 years.

Recommended Dosage: Iloprost is intended to be inhaled using the I-neb AAD System. The first inhaled dose should be 2.5 mcg (as delivered at the mouthpiece). If 2.5 mcg dose is well tolerated, dosing should be increased to 5.0 mcg and maintained at that dose; otherwise maintain the dose at 2.5 mcg. Iloprost should be taken 6-9 times daily (no more than once every 2 hours) during waking hours, as per individual need and tolerability. The maximum daily dose is 45 mcg (5 mcg 9 times daily). The 20 mcg/mL concentration is for those who repeatedly experience extended treatment times.

• Iloprost 10 mcg/mL or 20 mcg/mL solution should not be allowed to come into contact with the skin or eyes; oral ingestion of Iloprost solution needs to be avoided.
  
• Monitor vital signs while initiating Iloprost. Avoid initiating Iloprost in those with systolic blood pressure below 85 mmHg. Syncope can also occur in association with pulmonary arterial hypertension (PAH), mainly in association with physical exertion.
  
• Should signs and symptoms of pulmonary edema occur when inhaled Iloprost is administered in those with pulmonary hypertension, discontinue treatment promptly, as this may be a sign of pulmonary venous hypertension (PAH).
  
• Iloprost inhalation can induce bronchospasm. Bronchospasms may be more critical or frequent in those with a history of hyperreactive airways. Iloprost has not been assessed in patients with severe asthma, chronic obstructive pulmonary disease (COPD), or acute pulmonary infections.
