Xacduro is a co-packaged for intravenous (IV) use medicinal product containing sulbactam, a beta-lactam antibacterial and beta-lactamase inhibitor, and durlobactam, a beta-lactamase inhibitor, used in patients aged 18 years and older for the treatment of hospital-acquired bacterial pneumonia and ventilator-associated bacterial pneumonia (HABP/VABP), caused by susceptible isolates of Acinetobacter baumannii-calcoaceticus complex.
Limitations of Use: This medicine is not used for the treatment of HABP/VABP caused by pathogens other than susceptible isolates of Acinetobacter baumanniicalcoaceticus complex.
Recommended Dosage: Xacduro is a co-packaged therapeutic product containing sulbactam for injection and durlobactam for injection. The recommended dosage is 1 gram (g) of sulbactam and 1 gram (g) of durlobactam every six hours administered by intravenous (IV) infusion over three hours in adults with a creatinine clearance (CLcr) of 45-129 mL/min. Adjustments to the dosing regimen are recommended for patients with creatinine clearance less than 45 mL/min and for patients with creatinine clearance greater than or equal to 130 mL/min. The recommended length of therapy is 7 to 14 days. The length of therapy should be guided by the patient’s current clinical status.
- Severe and occasionally fatal hypersensitivity (anaphylactic) reactions and critical skin reactions may occur in individuals receiving beta-lactam antibacterial medicines. Prior to starting therapy with Xacduro injection, precise inquiry needs to be made concerning known hypersensitivity reactions to penicillins, carbapenems, cephalosporins, other beta-lactams, and other allergens. Stopt this therapy in case of allergic reactions.
- Clostridioides difficile-associated diarrhea (CDAD) may occur with the administration of almost all antibacterial agents, including Xacduro injections, and may range in severity from minimal diarrhea to fatal colitis. In case CDAD is confirmed or suspected, the risk/benefit of continuing this treatment should be evaluated.
- Prescribing sulbactam for injection and durlobactam for injection in the absence of a proven or strongly confirmed/suspected bacterial infection or a prophylactic indication is not likely to offer benefit to the individuals and increases the probability of the establishment of drug-resistant bacteria.
- Concomitant use with OAT1 inhibitors may be responsible for increased plasma concentrations of sulbactam. Concomitant use of OAT1 inhibitors (such as probenecid) with Xacduro medicine is not recommended.
