Zaltrap (ziv-aflibercept) is an FDA-approved anti-cancer medication given in combination with a chemotherapy regimen known as FOLFIRI for treating colorectal cancer that has spread. Administer the medicine before any component of the FOLFIRI regimen on the day of treatment.
- Treatment with Zaltrap 100 mg/4 mL or 200 mg/8 mL can increase the risk of severe bleeding. Patients should be monitored for signs of bleeding, such as unusual bruising, nosebleeds, or blood in stools. If severe hemorrhage occurs, Zaltrap treatment should be discontinued.
- This therapeutic drug may cause gastrointestinal perforations, which can be life-threatening. Symptoms like severe abdominal pain, fever, or changes in bowel habits should be reported immediately. If a perforation is confirmed, Zaltrap should be permanently discontinued.
- Ziv-aflibercept can affect the body’s ability to heal wounds. Patients undergoing surgery should stop taking this medicine at least 4 weeks before the procedure and should not restart until the surgical wound is fully healed.
- Ziv-aflibercept may cause high blood pressure. Blood pressure should be monitored regularly, and antihypertensive therapy may be needed to control it. Severe hypertension requires temporary or permanent discontinuation of the drug.
- Proteinuria (protein in the urine) is another potential side effect. Regular urine tests are recommended to detect this condition early. If proteinuria becomes severe, treatment with Zaltrap 200 mg/8 mL or 100 mg/4 mL should be temporarily stopped or discontinued.
- There is a risk of serious arterial thromboembolic events, including strokes and heart attacks. Patients with a history of these conditions should be closely monitored, and Ziv-aflibercept should be discontinued if severe events occur.
- Ziv-aflibercept can cause harm to a developing fetus. Women of childbearing potential should use effective contraception during treatment and for at least three months after the last dose. Pregnant women should be advised of the potential risks to the fetus.
