Panhematin (Hemin for Injection)

  • Medicine Name: Panhematin
  • API: Hemin for injection
  • Dosage Form & Strength: Single-dose Vial: 350 mg
  • Manufactured By: Recordati Rare Diseases Inc

Medical Uses

Panhematin is a hemin for injection used for amelioration of recurrent attacks of acute intermittent porphyria temporally related to the menstrual cycle in susceptible women, after initial carbohydrate therapy is known or suspected to be inadequate.

Limitations of Use: Before administering Panhematin, consider an appropriate period of carbohydrate loading (i.e., 400 g glucose/day for 1 to 2 days). This treatment is not effective in repairing neuronal damage because of the progression of porphyria attacks.

Recommended Dosage:

The recommended dosage of Panhematin is 1 to 4 mg/kg/day administered via intravenous (IV) infusion only for 3 to 14 days, depending on the clinical signs. The standard dose in clinical practice is 3 to 4 mg/kg/day. Repeat dose in more life-threatening cases, no earlier than every 12 hours. Do not exceed 6 mg/kg in any 24-hour period.

Warning & Precautions

  • Panhematin is administered intravenously. It may cause vein irritation or phlebitis. Monitor the infusion site carefully. Rotate injection sites if needed. Slow the infusion rate to minimize discomfort and prevent local vascular complications during treatment.
  • Panhematin contains heme, which can affect iron levels. Regularly monitor serum ferritin and iron parameters during treatment. Patients with iron overload or a history of hemochromatosis should be evaluated carefully before initiating therapy to prevent worsening iron-related complications.
  • Panhematin may alter blood coagulation. Monitor patients on anticoagulants closely, as dosage adjustments may be needed. Watch for unusual bleeding, bruising, or prolonged clotting times. Notify healthcare providers immediately if any abnormal hemorrhage occurs during therapy.
  • Panhematin can impact kidney function. Monitor renal parameters such as serum creatinine and urine output. Patients with pre-existing renal impairment should receive Panhematin cautiously. Adjust dosage or infusion schedule if renal function declines during treatment to avoid toxicity.
  • As Panhematin is derived from human blood, there is a risk of viral or bacterial contamination. Ensure it is obtained from screened donors and follow proper storage. Report any signs of infection immediately to prevent complications.
  • The safety of Panhematin during pregnancy is not fully established. Use only if the benefits outweigh potential risks to the fetus. Monitor maternal and fetal health closely. Avoid unnecessary exposure, and consult obstetric specialists for individualized care during treatment.
  • It is unknown whether Panhematin passes into breast milk. Exercise caution when administering to nursing mothers. Consider temporary interruption of breastfeeding or alternative feeding options. Monitor the infant for any adverse effects if therapy is necessary for the mother.
